This form is used to add or remove seats from your Hosted Dialer services.

By completing this form, you are signing up to have additional seats added, or existing seats removed.

Please note that any seats added, will have a bill date with today’s date, if you would like the bill dates to all be on the same day, please contact our billing department for assistance.

Your completed form will be forwarded to Hosted Dialer for processing automatically once you complete and submit the form.

If you complete the form during normal business hours, your login details will be emailed to you within the hour and a Hosted Dialer customer support representative will be contacting you shortly thereafter, to offer any assistance.
